Guest Post: If I had known then what I know now about aging-in-place

?Your mom will never go home again??

I wish I had known about aging-in-place 12 years ago when my 75-year-old widowed mother passed out in her Golden Valley, Minn., kitchen. When she came to and called 911, my modest Norwegian

mom refused to get in the ambulance until she could put on her pantyhose. On the way to the hospital, her heart stopped twice.

That evening, hundreds of miles away in North Carolina, my phone rang. It was her doctor, delivering the diagnosis I had hoped never to hear: ?Your mom will never go home again.?

If I had known then what I know now about aging-in-place, she could have lived her last three years in her beloved home. She may not have lived longer, but I know she would have been happier.

Instead, we moved her into the best assisted-living facility we could afford. The bills were a struggle, but the emotional cost to my mom was more devastating. She was forced to give up her cuddly cat,

her daylily garden, the hair stylist who knew all her grandchildren?s names, and the homemade cinnamon rolls at the local cafe. Typically, like us, people who face challenges living at home alone first consider moving into an assisted living facility (ALF.)

According to the National Center for Assisted Living in Washington, D.C., a no-frills ALF apartment costs at least $3,131 per month in Florida. The average length-of-stay is 28.3 months. Thirty-three

percent of ALF residents die there, and 59 percent are eventually transferred to nursing home care. The remaining 8 percent move in with family members or return to their private homes.

Perhaps the biggest surprise of all is that the median age of an ALF resident is 86.9 years, much older than the running and biking ?active seniors? commonly featured in ALF brochures and TV commercials.

While a Google search uncovers more than one million hits for ?aging-in-place,? most people I encounter have never heard the term. However, the expression has sounded loud and clear in dozens of American cities where pilot aging-in-place initiatives are underway, backed by local governments and respected organizations such as AARP and the Mayo Clinic.

Aging-in-place can significantly improve the quality of life?

My hope is that our local government and healthcare officials will be receptive to learning how aging-in-place can significantly improve the quality of life of local residents who have chosen to live their

twilight years here. While I?m too late to help my mom, since her death I?ve dedicated myself to helping Florida retirees remain in their homes long-term in spite of current or future illness, disability, or injury.

I?ve developed a six-stage comprehensive model for aging-in-place that makes it possible for most seniors to avoid transfer to assisted living or nursing home care despite declines in mobility, dexterity, strength, balance, vision, or hearing.

Successful aging-in-place combines home safety and fall prevention, home health services ranging from homemaker companions to specialty therapists and nurses, an understanding of how Medicare,

Medicaid, and insurance pay for aging-in-place, and local options for transportation and social activities so that aging-in-place does not lead to isolation and depression.

Too often, the decision about where you or your loved one will be living in the future is made in whispers outside a hospital room.

Instead, by planning ahead, seniors can make that decision themselves and preserve their dignity, independence, and community connections by exploring aging-in-place opportunities.

Mayo Clinic researchers have found that the cause of chronic sinus infections lies in the nasal mucus -- the snot -- not in the nasal and sinus tissue targeted by standard treatment. The findings will be published in the August issue of Journal of Allergy and Clinical Immunology and are available online at elsevierhealth.com/jaci.

"This strikingly teaches against what has been thought worldwide about the origin of chronic sinus infection: that inflammatory cells break down, releasing toxic proteins into the diseased airway tissue," says lead researcher and Mayo Clinic ear, nose and throat specialist Jens Ponikau, M.D. "Instead we found that these toxic proteins are released into the mucus, and not in the tissue. Therefore, scientists might need to take not only the tissue but also the mucus into account when trying to understand what causes chronic sinus infections and probably other airway diseases."

The findings could significantly change the way chronic sinus infection is treated, according to Dr. Ponikau.

"This has far-reaching implications," says Dr. Ponikau. "This suggests a beneficial effect in treatments that target primarily the underlying and presumably damage-inflicting nasal and sinus membrane inflammation, instead of the secondary bacterial infection that has been the primary target of treatments for the disease. Also, some surgeons have already started to change the way they do surgery for patients with chronic sinus infections, focusing now on removing the mucus, which is loaded with toxins from the inflammatory cells, rather than the tissue during surgery. Leaving the mucus behind might predispose patients for early recurrence of the chronic sinus infection."

Dr. Ponikau conducted this research along with Hirohito Kita, M.D., and Gail Kephart, Mayo Clinic allergic diseases researchers. While major basic protein was not distributed in the nasal and sinus tissue, the level of this protein in the mucus of chronic sinus infection patients far exceeded that needed to damage the nasal and sinus membranes and make them more susceptible to infections such as chronic sinus infection.

To conduct this investigation, Dr. Ponikau and fellow researchers collected specimens from 22 consecutive Mayo Clinic chronic sinus infection patients undergoing endoscopic sinus surgery. The surgeons extracted the maximum possible tissue and mucus during the sinus surgery. The surgeons also extracted tissue and mucus from healthy patients undergoing septoplasty, surgery to fix a deviated septum, for comparison with the specimens from the chronic sinus infection patients. Through various forms of laboratory examination of the tissue and attached mucus, the investigators observed an abundance of major basic protein throughout the nasal and sinus mucus in all 22 specimens, but not in the tissue.

Chronic sinus infection is one of the most common chronic diseases in the United States, affecting 32 million adults, according to the National Center of Health Statistics. Chronic sinus infection produces nose and sinus problems characterized by stuffy nose, loss of sense of smell, postnasal drip, nasal discharge, and head and face pain lasting three months or longer. It notably decreases the quality of patients' lives, impairing physical and social functioning, vitality and general health, according to the Mayo Clinic researchers.

Quality 'will win battle for US olive oil'


The United States olive oil market ? the worlds third-largest for consumption ? is poised to become a battleground between historic European producers and new challengers from California, Chile and Australia, according to a new study from Rabobank Food & Agribusiness Research and Advisory.

US producers are expected to capture 5% of the overall US olive oil market over the next five years. They also look set to take around 10% of the premium extra virgin segment provided they can tighten US olive oil quality labelling standards and educate the consumer on the merits of high quality olive oil.

Large Spanish and Italian companies which currently dominate global production and the US market, will need to focus on quality in order to counter the challenge by New World producers of premium oils.

?US olive oil producers have a good opportunity to build a premium US market niche by promoting their efficient, high quality production and California?s strong local label appeal,? said the report?s author, Karen Halliburton Barber, assistant vice president and senior agricultural analyst with the Food & Agribusiness Research and Advisory group in California.

?Spain will continue to be the price setter in the global olive oil industry. Responding effectively to the challenge from New World olive oil producers will require much focus on quality and production efficiency by European olive oil players,? added the report?s co-author, analyst Vito Martielli, Senior Associate Grains and Oilseeds Food & Agribusiness Research and Advisory in Europe.

Market growth is attracting new entrants. The US retail olive oil market has grown to sales of $1.11bn in 2010, helped by labelling that highlights health benefits of consuming monounsaturated fats. US retail olive oil sales are forecast to rise 13% between 2010 and 2015, growing at 2.5% a year.

Imports currently account for more than 99% of the market. Italy has a 51% share; Spain, the world?s largest olive oil producing nation, has 23%, Morocco 9% and Tunisia 8%.

European brands dominate. Three European nations, Spain, Italy and Greece, account for 70% of global olive oil production. As in the global olive oil trade, most US imports are provided by Spain-based Deoleo (Bertolli and Carapelli brands); Italy?s SALOV (Filippo Berio); US-based Pompeian; Spain?s BORGES Group (Star); Sovena Group of Portugal and Spanish-US joint venture Hojiblanca-Cargill. Some of these players also pack private labels meaning they can claim almost a quarter of the market. High-end, niche Italian producers dominate in the top market segments of extra virgin olive oil.

Californian, Australian and Chilean producers are now challenging. Producers in California, Australia and Chile use modern, high-density, mechanised production techniques to produce top-quality extra virgin oils. Selling premium oils differentiates their products and covers higher production costs. Already California has 12,146 hectares of intensive olive trees, and planting is expected to add 2,024 hectares a year through 2020.

Premium olive oil production is well established in Australia, where producers have captured market share from imports. A model for the US industry, it now exports to the US. Chile too has established a high-tech premium oil industry, with growing production and exports.

High quality extra virgin oils are seen as having greater health benefits. Though a global standard for olive oil exists, overseen by the International Olive Oil Council, Australia has developed a national standard and the US may follow suit, adding to the attractive image of Californian production. Chile too is working on a law to regulate olive oil grades, labelling and origin.

A focus on quality enables New World producers to target the premium segment in the US, where more than half of bottles of oil sold in US supermarkets are labelled extra virgin.

Overall, European exports look vulnerable. Though much Italian oil is perceived of high quality, oil from Spain typically commands lower prices and is seen as of lower quality.

European producers must respond strategically. New World countries and growing Middle East and North African (MENA) countries are now challenging the traditional dominance of European producers in olive oil. Success in the US market will accrue to those who can demonstrate the quality of their products and persuade consumers to pay top prices for them. Niche players can build on their brands by producing top-quality extra virgin oil that can command premium prices. Big producers must also focus on quality, but should target large retailers and private label vendors.

Ellington, Hopkins lead No. 14 Clemson past Auburn

ATLANTA (AP) ? Sammy who? Andre Ellington and DeAndre Hopkins made sure Clemson didn't miss its star receiver.

With Sammy Watkins back home serving a two-game suspension for a drug arrest, the No. 14 Tigers still showed plenty of offense in their opener Saturday night. Ellington rushed for 231 yards, while Hopkins set a school record with 13 receptions, including the game-winning touchdown in the fourth quarter for a 26-19 victory over Auburn.

The fast-break offense of coordinator Chad Morris carried on just fine without Watkins, piling up 320 yards rushing and 528 yards overall.

"We had to fill that void collectively as a group, and we did that," coach Dabo Swinney said. "Guys stepped up, and filled the void for Sammy. When he's on the field, obviously he can impact the game. But we're proud of how the team picked up the slack."

Playing before a 50-50 crowd at the Georgia Dome ? basically halfway between the two campuses ? Clemson shook off any hangover from last season's embarrassing 70-33 loss to West Virginia in the Orange Bowl, though it wasn't easy.

The second half was a battle of field goals before Tajh Boyd lofted a 4-yard touchdown pass to Hopkins, who made a brilliant, twisting catch in the corner of the end zone with 9:17 to go. The junior finished with 119 yards receiving.

"I was in a great zone, like I always am," said Hopkins, who broke the record of 12 receptions set by Airese Currie against Middle Tennessee State in 2003. "Coach Morris had a great game plan to get playmakers the ball and spread it around, and that's what he did."

"No one is celebrating more than Sammy Watkins back in Clemson," Swinney added.

Ellington broke off a couple of long runs to set up a chip-shot field goal with just over a minute remaining for the final margin. Auburn got the ball back one more time, but with only one timeout. The Southeastern Conference team didn't even get it across midfield, the game ending on a desperate heave by new quarterback Kiehl Frazier.

The 5-foot-10 Ellington scooted around and through the Auburn defense on 26 carries, the longest of them a 68-yarder that set up Clemson's first touchdown.

"I certainly didn't get it all on my own," Ellington said. "I'm just thankful to have a great line in front of me."

Boyd did his part, too, completing 24 of 34 for 208 yards and carrying the ball 19 times for 58 yards. Clemson made life miserable for new Auburn defensive coordinator Brian VanGorder, who returned to the stadium where he spent the past four seasons with the NFL's Atlanta Falcons.

"Our credit goes to the guys up front," Swinney said. "When you rush for 320 yards and win the turnover margin, you're going to win a bunch of ballgames."

Frazier had a 54-yard touchdown pass to Emory Blake, hitting the receiver in stride to put Auburn ahead 7-3 late in the first quarter. The sophomore, who beat out last year's starter, Clint Mosey, for the job in fall camp, showed plenty of potential but is still a work in progress. Frazier passed for 194 yards despite completing just 11 of 27.

"I did all right," Frazier said, "but I made a lot of mistakes and left some plays out there on the field."

Tre Mason led Auburn in rushing with 106 yards.

Both teams kicked four field goals. Cody Parkey connected from 37, 46, 27 and 36 yards for Auburn, the last of them putting his team ahead with 12:50 remaining. Chandler Catanzaro was just as accurate for Clemson, booting it through from 24, 40, 37 and 18 yards.

On the first play of the fourth quarter, Boyd threw behind tight end Sam Cooper, the ball bouncing off his hands and right to Auburn linebacker Daren Bates for an interception. He returned it to the Clemson 35.

Mason ran it three times, taking the ball inside the Clemson 10, but a crucial holding penalty on Greg Robinson, a redshirt freshman making his first college appearance at left tackle, cost the Tigers a chance for the second TD. They wound up settling for yet another field goal by Parkey.

That wasn't enough to hold off Clemson.

"Turnovers and not taking advantage of red-zone opportunity was the main difference," Mason said.

Boyd broke off a 27-yard run after faking a handoff out of the shotgun and scrambled for another first down on third-and-5, keeping the drive going with a 10-yard gain. Ellington slid around right end for 8 yards, then took off up the middle for a 9-yard run. Finally, on first-and-goal at the Auburn 4, Clemson stuck it in the end zone.

Credit Hopkins for that. Boyd took one step back and floated a pass toward the corner. Even though cornerback Chris Davis grabbed at the junior receiver, drawing a flag, Hopkins went up and got it. He corkscrewed his body around to snag the ball, managing to get a foot down before he tumbled out of bounds.

Frazier had a second-quarter interception that was essentially a punt, lofting a desperation pass down the field on third-and-15 that was picked off by Rashard Hall way down at the Clemson 15.

Turns out, that just gave Ellington more room to run. He burst down the field on his 68-yarder, and the officials tacked on a few more yards when Jonathan Mincy was flagged for a personal foul at the end of the play. On the very next snap, Roderick McDowell scored on a 7-yard run to put Clemson ahead 13-7.

Clemson gave the maligned Atlantic Coast Conference a much-needed win over an SEC opponent on the second of back-to-back openers at the Georgia Dome between the two leagues. Tennessee romped past North Carolina State 35-21 the previous night, a game marred by the death of a 20-year-old fan who fell out of the upper deck.
